[Tool] I made a bad-to-good habit chart!

I basically sat down and listed the 5 most pertinent, yet fixable problems in my life right now, and then listed their opposite good habits in green. Every night, before bed, I'll take 1 minute to go through each bad-to-good habit item and put an 'X' if I did the bad habit that day, or a check if I did the good habit, and I'll continue this for 30 days.

At the end of the thirty days I'm going to post up the results of what happened. No bullshit. No playing around. If I succeed then that's good, but even if I fail and nothing changes I'll still show the data and reflect on what the hell I learned.
